  • Complete the student section of the form
  • Go to your guidance counselor so they can verify that you are eligible to use the form.
  • Make sure your guidance counselor affixes the high school’s raised seal or stamp on the form.
  • Mail the form directly to the admission office of the college or university to which the student is seeking admission.
